چکیده

Sir,—According to Alexopoulus, Larson and Lindholm (1983), the airway aerogram obtained with x-rays is easily identifiable along its anterior aspect, from the epiglottis to about 60 mm below the vocal chords. Based on this evidence, and to avoid problems arising from technical difficulties during blind nasotracheal intubation, we have successfully used an x-ray image intensifier to guide the introduction of the tracheal tube between the vocal chords. We place the patient in a supine position, the head resting on a pillow 8-10 cm above the plane of the operation table. After good topical anaesthesia, the image intensifier is positioned so as to obtain a lateral x-ray view of the oropharyngotracheal aerogram. In this manner it is easy to identify basic anatomical structures such as hyoid bone, epiglottis, Morgagni's ventricles and the trachea. Once these elements are recognized on the screen, the endotracheal tube is inserted in the correct position. We consider that this technique can be useful during difficult blind nasal intubation.

Anaesthesia in Upper Respiratory Obstruction.

The causes of upper respiratory obstruction are reviewed. The symptoms and signs are graded according to severity. It is stressed that cyanosis indicates very severe obstruction, and that stridor at rest is a very informative local sign of severe obstruction. In severe obstruction respiratory depressant drugs and atropine are contraindicated while helium may be helpful. When general anaesthesia...

Tracheal tube cuff inflation as an aid to blind nasotracheal intubation.

We have assessed the efficacy of tracheal tube cuff inflation in the oropharynx as an aid to blind nasotracheal intubation in 20 ASA I and II patients undergoing elective oral surgery. The trachea was intubated once using the technique of tracheal tube cuff inflation in the oropharynx and once keeping the tracheal tube cuff deflated throughout the manoeuvre. With the cuff deflated, intubation w...

A survey of nasotracheal intubating skills among Advanced Trauma Life Support course graduates.

The American College of Surgeons' Advanced Trauma Life Support procedure teaches that blind nasotracheal intubation should be performed in the presence of a suspected or proven cervical spine injury in an unconscious but breathing patient who requires an artificial airway. We studied a group of non-anaesthetically trained graduates of the Advanced Trauma Life Support course and examined their s...

Nasotracheal intubation: look before you leap.

Nasotracheal intubation is frequently used for airway management during maxillofacial surgery. Complications such as haemorrhage occur more frequently with this route of intubation than with the orotracheal route. This case report describes a male patient aged 51 yr who developed severe epistaxis after the tube had passed the nares. As an additional complication laryngoscopy and endotracheal in...

Complications of prolonged nasotracheal intubation in children.

The immediate and late complications of prolonged nasotracheal intubation in eightythree children are reviewed. Twenty-six of the thirty survivors were examined by laryngoscopy to determine the incidence of permanent laryngeal lesions. Seventeen had no lesion. Of the remaining nine, two had severe subglottic stenosis, four slight subglottic stenosis, and three minor degrees of scarring without ...
